2021 Spark of Love Toy Request Form

The Fountain Valley Fire Department is partnering with the Fountain Valley Community Foundation to distribute toys to nonprofit and community organizations in Fountain Valley this holiday season. Toys will be distributed to nonprofit organizations and community groups within Fountain Valley.

 If you are a nonprofit or community organization, please complete one of the following:

  1.  Toy request form including your 501(c)3 Tax ID (if applicable) number for verification and send to christie.araiza@fountainvalley.org; OR
  2. Visit FVcommunityfoundation.org to submit a Toy Request Application online

While the Fountain Valley Community Foundation will do our very best to meet all the needs of the community, please know that there is no guarantee that you will receive toys. We appreciate your understanding.
